Medical Forms

The links below will provide the instructions and forms necessary to be medically eligible to participate in intercollegiate athletics at EMU for the 2012-13 academic year.  To view this pdf document, you will need the free Adobe Acrobat Reader.  Be prepared to print multiple pages.  If you need a paper copy mailed to you, please contact Lucinda Schlabach at (540) 432-4440.

 

First Year Athletes and Transfers:

You must complete the Student Health Center Undergraduate Health Evaluation form AND the EMU Athletic Medical Eligibility forms to be eligible for practice and competition. The forms are not the same and both must be completed and found acceptable.

Spring registration due date for Health Center Undergraduate Health Evaluation form: Dec.  5, 2012

Winter and spring sports due date for EMU Athletics Medical Eligibility form: one week prior to team non-traditional season or conditioning sessions

 

Returning Student-Athletes: 

EMU athletes are required to complete the EMU Athletic Medical Eligibility Forms annually. The athletic medical eligibility forms for returning athletes do require a completed medical history with physician’s review of the medical history and annual physical examination.  To be accepted, returning athlete pre-participation physical exams must be conducted after July 1, 2012.
