2018-19 Athletic Medical Eligibility Instructions
All EMU athletes participating in NCAA sports are required to complete the EMU Athletic Medical Eligibility process annually.
The link below will provide the instructions to be medically eligible to participate in NCAA intercollegiate athletics at EMU for the 2018-19 academic year. To view this pdf document, you will need Adobe Acrobat Reader.
- First-year and transfer athletes will complete the EMU Health Services Health Evaluation Forms and the web-based SportsWare information.
First-year and transfer athletes may obtain their pre-participation physical examinations after May 15 and should return their Health Services forms by June 15. You have until July 1 to complete the SportsWare Athlete portal process. Click here for the instructions for first-year and transfer athletes.
- Returning athletes are required to complete their updated medical history document as well as complete an annual physical examination. The physician must sign both documents. These documents can be found in the Forms section in your SportsWare profile page.
Returning athlete pre-participation physical examinations must be conducted after July 1 and be returned by August 10 for all athletes. Click here for the instructions for returning athletes.
- By NCAA rule physical examinations conducted after July 1 but more than 6 months prior to any athletes first team practice or team conditioning will require a review by the team physician for approval.
